In about one fifth of all known cases of healthcare fraud, consumers are in terms of the perpetrators, according to the insurance association. All but many of the rest involve providers.

Some investigators also are convinced that private capitated plans are being charged for excessive clinical services and testing for some hospital emergency departments, which can bill them separately.
